The Lark Diabetes Prevention Program is offered at no additional cost to you as part of your UnitedHealthcare® Medicare Advantage plan coverage. Eligibility requirements include qualifying as a person with prediabetes according to a survey designed by the Centers for Disease Control. Participants in the program receive a digital scale to help track their progress during the program period. Eligibility determinations are made by Lark at its sole discretion.
